Form for registering deviations. This form can be accessed from within the deviation system for people who can log in to the system. Users who do not use the system can also submit deviations via a URL address. This can be found under ‘Deviation management’ in the system settings. See separate description. Desktop computers and tablets/mobile phones can be used to submit deviations.

Email:/Name:/Tel.: Contact information of the reporter. Email address is compulsory. The email address and name automatically come up from a logged-on user. Tel. no. and department will also be shown here if they are registered for the user.

Type: Select type of deviation. The deviation will be sent to the person who has been set up as the supervisor (Administrator) for this type of deviation.

Origin: Option to indicate what the origin of the deviation was. Filter options are available.
Category
One or more categories can be linked to the deviation (Select multiple by holding down the CTRL key). Filter options are available.
Description
Insert a more detailed description than you can insert in the subject. Searchable.
Immediate action
Insert an immediate action that has been performed.
Suggested corrective actions
Insert suggestions for any corrective actions.
Attachment
Insert/upload up to 3 pictures/files.
Send message
When the deviation has been sent, you will receive a receipt that the deviation has been sent, as well as an email with a link to the deviation, allowing you to follow its status. People logged into the system also have a full overview in ‘My registered deviations’.
